Alex takes the lead as a violent young prisoner is rushed into Hope Zion -- cuffed and restrained. Unwilling to stereotype her patient, Alex gives her the benefit of the doubt and with Gavin's help, they review her medical records and discover she's been given an intense cocktail of drugs to make her compliant.
When the drugs are flushed out her, the prisoner's true character comes through and Alex, Gavin, and Shahir are able to properly diagnose and treat her. Meanwhile, Joel steps out of Hope Zee to take Dana Kinney out for lunch in an attempt to lure her back to his surgical team for a special case.
After much deliberation and persistence from Chief Goran, Dana gives in to operating on a refugee with facial paralysis because of his heartbreaking story, but is hesitant to return to the hospital that passed her over for Chief of Surgery.
Down in the morgue, Charlie is about to harvest bones from a deceased donor and is surprised when the patient's Spirit is a familiar figure to the doctors at Hope Zion - especially Maggie. This week, both Alex and Joel must become more than doctors -- they must become advocates -- in order to save their patients.
