Sam was surprised to see Diana back in the hospital. Diana looked weak and notified Sam that her cancer was spreading. Sam was also shocked to hear that Diana had refused any more chemotherapy. She refused any medicine, monitors or blood tests. She was determined to go to Costa Rica to sit on the beach.
Kovac was faced with some difficult decisions since the ER was forced to close down because of some much needed renovations. He had to re-assign the ER staff to different departments of the hospital. The hospital staff was more concerned about their jobs, especially when Kovac had sent all of them invitations to attend a formal and mandatory dinner for the hospital.
Sam searched for Diana throughout the ER and found her scouring the internet in the ER lounge. Diana was frantically trying to find a ticket to Costa Rica and Sam sat her down and made her rest. Sam tried to get Diana to change her mind and accept the hospital's help to get better.
Gates and Morris were arguing on how to treat a patient. Morris stood by his decision and when he left the room, Gates convinced Abby to perform the treatment his way. Morris was less than thrilled to walk back in later on and see what Gates had done. Infuriated, Morris told Gates to go home and also threatened to write him up for his rebellious behavior.
Ray was caught in an awkward moment when he was forced to work with Katie. She wondered why he never called after they slept together and he tried to explain his actions to no avail. He still came out looking like a sleaze. She accused him of dumping her to pursue Neela and wanting nothing to do with him.
Kovac told Morris he did the right thing by sending Gates home and writing him up, even when Gates was in the right. Morris told Kovac that Abby deserved to be written up for going with Gates' procedure. She challenged Morris to write her up and he decided not to. But, he warned her not to test him.
Sam made an agreement that she would help Diana with all of her travel arrangements to Costa Rica if Diana agreed to get some medical help for her illness. Her vitals began crashing while Kovac and Ray were treating her. And there was nothing that could be done because she requested to be a DNR. When her vitals normalized on their own and she became coherent again, Diana pleaded for the doctors to stop working on her.
Sam was angry with Kovac because he had broken Sam's agreement and re-admitted Diana. Sam couldn't stand the fact that Diana could possible die in their hospital. Sam offered to take Diana home. Diana confessed to Sam that she didn't have any kids or friends and she was scared of dying alone.
Pratt was excited to see Chazz working as an EMT at the hospital. Things took a dramatic turn when later that day, Chazz was taken into the ER for alcohol poisoning. Pratt was furious to find out that Chazz took 15 shots of tequila as an EMT hazing. Pratt stopped Morris from tubing his brother just as Chazz was able to apologize to Pratt.
Sam helped Diana get home and situated. The two began to talk about life and choices that are made throughout it. Diana encouraged Sam to live and travel more, because she was lucky enough to have time on her side. Later on, Sam took the camera that Diana had given her and took pictures of Diana with it.
Ray was able to sneak away some time to talk to his patient's friend about jealousy. Ray was able to think about Neela and dealing with her and Gates' relationship.
Gates came home early and was followed by his dad and Sarah who were at the video store. Gates sent Sarah to her room so he could talk privately with his father. Gates was extremely upset to discover his dad had been drinking while driving Sarah around and ordered him out of the house. Gates and his father got into a physical fight outside the apartment as Gates was throwing out his belongings.
The time had come for the ER to shut down for renovations. All patients were being moved out of the hospital. Kovac was becoming more and more anxious with not knowing the future of where the ER would be. Kovac just wanted to be a doctor and in the end, he sent his resignation in as ER chief.
