AceShowbiz - ABC is set to broaden the Grey's Anatomy universe once again with a fresh spinoff centered in rural West Texas. The new medical drama series has received a straight-to-series order, as confirmed by Deadline on Tuesday, May 19. This marks another expansion under the guidance of Shonda Rhimes and Grey's Anatomy showrunner Meg Marinis, who will co-create, write, and executive produce the project.
Joining them as an executive producer is Ellen Pompeo, the iconic star of Grey's Anatomy. The upcoming series is slated to debut midseason in 2027 and is described as an "edgy drama about a team at a rural West Texas medical center – the last chance for care before miles of nowhere," according to Deadline. This will be the fourth installment in the franchise, following the earlier spinoffs Private Practice and Station 19.
Unlike previous spinoffs that focused on existing characters from Grey's Anatomy — such as Kate Walsh’s Addison in Private Practice and Jason George’s Ben in Station 19 — this new series will center around an entirely new group of doctors. However, it is expected to maintain ties to the original show, potentially through characters like Debbie Allen’s Catherine Fox.
The announcement arrives amid significant cast changes on Grey's Anatomy itself. Earlier this year, the show experienced notable exits, including the death of Monica (played by Natalie Morales) and Amelia (portrayed by Caterina Scorsone) opting for a sabbatical.
Showrunner Meg Marinis expressed enthusiasm about the new project, stating, "I am incredibly excited to expand the Grey's Anatomy universe. This opportunity will bring new characters and stories to life that will embody the same heart, emotion and connection audiences have loved from Grey's for more than two decades – all set in my home state of Texas." She added gratitude to Shonda Rhimes for creating the dynamic world that the spinoff will build upon.
It remains undecided if Marinis will oversee both Grey's Anatomy and the new spinoff simultaneously. Her predecessor, Krista Vernoff, previously managed both Grey's Anatomy and Station 19 at the same time.
The recently concluded Season 22 saw several cast shakeups. Caterina Scorsone’s Amelia took a break following the early-season death of Monica. Reports have indicated that Scorsone's hiatus may extend until the end of 2025, partly due to network cost-cutting measures. Additionally, the show reduced the minimum episode guarantees for veteran cast members from 18 to 14 episodes per season, contributing to some actors missing multiple episodes.
On the topic of cast changes, Meg Marinis previously hinted that not all characters would remain safe as the storylines evolve. In an interview with The Hollywood Reporter in May 2025, she acknowledged the necessity of reducing the cast due to industry-wide budget constraints. Marinis remarked, "If we can get the writers upset then we know we're doing it right. It's not as satisfying a story if people aren't upset when someone leaves." She also noted that she has been open to altering plans based on feedback.
Before the conclusion of Season 22, notable departures included Kim Raver and Kevin McKidd. Despite these exits, the series continues to stream on Hulu, maintaining a strong viewership base.
With the new spinoff set in a distinct rural setting and focusing on fresh characters, ABC and the creative team aim to inject new energy into the beloved franchise. The series promises to deliver the emotional depth and compelling storytelling that audiences have come to expect from the Grey's Anatomy brand, while exploring the unique challenges faced by medical professionals in a remote West Texas community.
This development signals a strategic move by ABC to rejuvenate the franchise amid cast transitions, ensuring that Grey's Anatomy remains a staple of medical drama television for years to come.
