J.Lo filed the divorce petition from Ben on Tuesday, August 20, coinciding with the second anniversary of their wedding. In the petition, she included a request to restore her maiden name after the divorce is granted.

Sources close to the "Atlas" star later revealed to PEOPLE that she is "upset and disappointed" with the actor.
