The original track explores Charli's insecurities and mental health battles. On the remix, Grande shares her own experiences with speculation about her character and appearance.
Grande sings, "It's a knife when they know they're counting on your mistakes / It's a knife when you're so pretty, they think it must be fake / It's a knife when they dissect your body on the front page / It's a knife when they won't believe you, why should you explain?"
She has previously confirmed getting cosmetic procedures like botox in the past, but has denied some of the rumors surrounding her appearance including boob job and BBL.
Charli explained her choice of Grande for the song, saying, "I'm definitely finding more than ever that my words are being picked apart, taken out of context. [Grande] definitely knows this feeling more so than me."
Lorde is featured in another remix. Like Grande, she shared her struggles on a remix of Charli's "Girl, So Confusing." The two singers revealed they had a complicated relationship, partly due to physical comparisons and insecurities. Lorde expressed feelings of "trapped in the hatred" and revealed dealing with body image issues.
The remix of "Sympathy is a Knife" appears on Charli XCX's album "Brat and It's Completely Different But Still Brat," released on October 11. Charli debuted the album at a listening party at Storm King Art Center in New York, where attendees enjoyed pizza, sliders, and Brat-themed drinks. The album's tracklist was displayed on a massive art installation, and the event was live-streamed on Twitch.
