Ray asked Pratt how Neela was doing since her husband died. He wanted to attend the funeral with Pratt and Neela, but Pratt told him to stay behind. When Pratt arrived to pick Neela up, she was not ready to go. She told him she wasn't ready to say goodbye and move on. Pratt convinced her to go.

While the two were getting ready for work, Kovac proposed marriage to a surprised Abby. She didn't have a definite answer for him and Kovac insisted on giving her the silent treatment at work for the rest of the morning.

An EMT trainee, Mary, was assigned to shadow Sam at work. Sam tried to be patient as she helped Mary fill out charts and check on patients, but Mary was not taking her responsibilities seriously. Things escalated when she was clumsy with her CPR on an elderly patient and Kovac had to push her out of the way to save the man's life.

Sam's ex-husband, Steve, was brought in after a prison fight with a fellow inmate. The other man was brought in as well. Sam and Mary took the other guy's vitals and realized he was only slightly wounded from the fight. Steve, on the other hand, had torn the stitches from his recent appendicitis.

Neela attended Gallant's funeral with Pratt. She was upset that Gallant's father approved of a military funeral without her consent. After the service, she told him that Gallant would still be alive if it wasn't for his family urging him to return to Iraq.

Mary, Steve, and his fellow inmate, Hendrix, took the ER hostage. They managed to knock the guard unconscious and take his gun. From there, they took Sam hostage and demanded she find a way to help Steve and Hendrix escape without being noticed. Kovac walked into see what was wrong and Mary injected him with a paralyzing drug.

Sam tried to help a paralyzed Kovac and directed Steve to help her. Hendrix and Steve told Mary to get the van and they would be out shortly. Abby sensed something was wrong, but Mary distracted her. Hendrix pointed the gun at Sam and demanded she go with him and Steve. She had to leave Kovac on the operating room table.

Sam escorted Steve out in a wheelchair and Hendrix was dressed in a guard's uniform. She didn't make any eye contact with her co-workers as she walked past. Abby knew there was something wrong and one of the hospital guards tried to stop the trio from leaving. Just then, Steve and Hendrix pulled out guns and started shooting the place up.

Steve dragged Sam to the getaway van. She pleaded with him to think about their son; he assured her he already had. He opened the van door and their young son was tied up and gagged in the back of the van. Sam climbed in and they sped away with Mary behind the wheel. Meanwhile, the rest of the ER was recovering and trying to help those who had been shot, including a guard and Jerry. Jerry was beginning to fade and the doctors couldn't stop his bleeding.

During the shootout, Abby had hit her head and was knocked to the floor. She quickly bandaged up and began assisting with gunshot victims in the ER. While she was operating, she began to get dizzy. She asked Morris to take over working on Jerry and left to get more supplies. She was also worried because she hadn't seen Kovac.

Neela wanted advice from Pratt. She didn't know how to move on without her husband. Pratt assured Neela that Gallant would have wanted her to move on and be happy. He was interrupted with an emergency phone call from the hospital.

Kovac was still tied down to the operating table. An unknowing Abby was about to enter the room in search of supplies when she stopped and doubled over with intense cramps. Kovac saw her face in the window, but because of the chest tube Sam had inserted in his mouth earlier, he couldn't speak. He tried to get Abby's attention, but couldn't. Abby leaned down and noticed she was bleeding; she got dizzy and passed out. Kovac saw the handprint of blood she left on the window, but couldn't do anything.
